In the bolus fluid trial, the hemoglobin and hematocrit values (mean +/- SEM) decreased by a maximum of 1.5 +/- 0.1 g/dL and 4.1 +/- 0.3% at 1 hour. There was no difference in hemoglobin or hematocrit during the no fluid or maintenance fluid treatments. No significant changes occurred in white blood cell or platelet counts.

RESULTS: Three groups were clinically similar regarding blood loss, mean arterial pressures and coagulation parameters. But allogeneic transfusion requirements were significantly less in hemodilution groups (20% in ANH, 40% in HHD) compared to the control group (100% of patients). CONCLUSION: We conclude that hemodilution (both ANH and HHD) decreases the demand for homologous blood without adversely affecting hemodynamics or coagulation parameters and HHD seems to be a simple and valuable Clinically, the most noticeable effect of hemodilution is a marked reduction in the perfusion pressure at initiation of CPB. Compared with baseline, hemodilution during cardiopulmonary bypass resulted in an average 35% ± 7% decrease in the concentration of coagulation and fibrinolytic proteins, whereas blood loss was responsible for an average 6% ± 5% decrease in these proteins. A low hematocrit may result from an increased plasma volume (hemodilution) or from reduced red blood cell volume (true anemia).

CONCLUSIONS: Increased anesthetic depth with halothane or ketamine resulted in a decreased tolerance to acute anemia, as reflected by a significant increase in critical hemoglobin concentration. PMID: 12826848 Hemodilution reduces the need for blood transfusion and hereby the risk for transmission of infectious agents. The present study investigated effects on systemic and myocardial circulation and oxygenation, and blood lactate concentrations; induced by nitrous oxide, isoflurane, hypoxia, and uncompensated blood loss, in pigs hemodiluted to a hematocrit of 11%. In both groups the change in blood flow velocities is comparable. For the first time the results of relative changes in blood flow velocities following hemodilution and retransfusion in healthy volunteers are described that correspond closely by relative cerebral blood flow alterations found in animal studies as well. Hemodilution can be normovolemic, which implies the dilution of normal blood constituents by the use of expanders.

The aim of the present study was to evaluate the effect of the hypervolemic hemodilution of 6% hydroxyethyl starch 130/0.4 on the EC50 of propofol at two clinical endpoints. (c) Resistance effects of hemodilution with plasma or after heparinase treatment agree with model predictions based on tube flow theology of blood.

The effects of hemodilution during CPB on ischemic neurologic injury have been studied in both animals and humans. Although early animal studies indicated a potential benefit to hemodilution, strengthening the belief that hemodilution during cardiac surgery was without consequence, 13,14 most of these studies were limited in that they did not mimic CPB or reduce hematocrit below 30%.

Abstract. This study evaluates the effect of blood volume and hematocrit changes on brain tissue during temporary global ischemia. Normal saline was administered intravenously to 55 gerbils to achieve hypo-, normo-, and hypervolemic hemodilution and uniform 30% hematocrit reduction. This study evaluates the effect of hemodilution by various common resuscitation fluids, and the efficacy of activated recombinant factor VII (rFVIIa) on coagulation parameters in human blood in vitro.Samples from normal healthy volunteers (n = 9) were hemodiluted from 0% to 90% with normal saline, or 0%, 40%, 60%, and 80% with 5% albumin, Hespan, Hextend, normal saline, or lactated Ringer's In the bolus fluid trial, the hemoglobin and hematocrit values (mean +/- SEM) decreased by a maximum of 1.5 +/- 0.1 g/dL and 4.1 +/- 0.3% at 1 hour.

Hemodilution effect

Severe hemodilution during cardiopulmonary bypass (CPB) is a risk factor for morbidity in cardiac surgery. This study investigated the effects of different degrees of hemodilution with CPB on post-CPB TEG parameters and coagulation-related outcomes. Hemodilution has the largest effect on drugs whose distribution is primarily confined to the bloodstream. The impact of hemodilution is less for drugs with a large volume of distribution because drug diluted in the bloodstream may be replaced by drug stored in the tissue. Further, the impact of hemodilution is inversely related to age. RESULTS: Three groups were clinically similar regarding blood loss, mean arterial pressures and coagulation parameters. But allogeneic transfusion requirements were significantly less in hemodilution groups (20% in ANH, 40% in HHD) compared to the control group (100% of patients).
